Sagecare 147 Elder Street brings to bear all the latest innovations and approaches in memory care building design and compassionate care, in a setting focused solely on dementia care. It’s a beautiful building both inside and out, and there’s also delicacy in the care given to residents here, administered by devoted staff, specially prepared and trained to help them. Over 20 years of experience is brought to bear in every nuance of living here.

For both families and those suffering from Alzheimer’s Disease and related forms of dementia, the cognitive decline and associated symptoms can be a cause of great distress and confusion. The person with dementia suffers natural frustrations attendant with this disease, but the setting and the gentle care staff at Sagecare alleviate those. Staff understand negative behaviours associated with dementia (loss of judgement, mood changes, etc.), looking at things through the eyes of empathy. They’re “on” all the time, trained to consider situations from that viewpoint.

In addition, residents have opportunities for ongoing personal growth at Sagecare, surrounded by the elegant, brightly lit, well-appointed setting. The focus here goes beyond dealing with dementia to living life as richly as possible. People are treated with class and distinction. Families will find that they can reconnect and recover relationships with loved ones. Seniors with dementia to feel at home with themselves. Staff’s expertise and the carefully created environment elevate dementia care.

Who are you as a community?

Located in a residential neighborhood across the street from a park, 147 Elder St. provides a home like environment exclusively for dementia care.  In our community, dementia is the rule not the exception.  It’s just life. Working with elders experiencing dementia for over 20 years, we know that there is virtually no situation in which their condition can be called stable, simple or predictable.  Our skilled nurses can distinguish between emerging medical problems and evolving behavioral changes ensuring all our elders get the care they need. Every member of our community is well-known, honored, respected and loved.

What do you do differently or uniquely well?

With over 20 years of experience in providing dementia care, Sagecare stands unique as one of the few purely dementia care retirement homes in the GTA. Everything from our physical layout and lighting to our holistic approach to care and therapeutic activities is tailored to those living with dementia.

Our residents spend their days on our main floor enjoying therapeutic recreation activities, exercise programs, music therapy, art therapy, and a wide range of other programming tailored to their abilities. Our experienced staff partner with family members to provide truly person-centered care and programming.

What supports do you offer residents as they adjust to life there?

Prior to move in Sagecare works extensively with families to better understand the future resident. Families help to establish day-to-day routines, preferred foods, activities, and music, and known triggers. This information is used to structure the resident's daily routine and activities.

Upon admission Sagecare provides overnight a one to one personal support worker overnight for the first two nights to better understand the resident's sleep patterns and ensure they're safety.

We understand that visits can be difficult for families during the transition period. Our team facilities visits with the new resident to help both families and residents feel more at ease with the move.

What might residents find surprising about your community?

Sagecare recognizes that dementia has tremendous effects on the family of the resident and they require support as well. Sagecare has an onsite social worker who works with families, holds monthly support groups, and provides one-to-one therapy and counselling to our families.

Our core team of nurses, personal support workers, dietary staff, and housekeepers have been with Sagecare for over 10 years. It's this experienced team of staff that helps take the care element away from families and give them the opportunity to be a loved one.

We also have a robust family council who helps our new families with integrating into our community, and supports them through the transition.

COVID-19 Information

At this difficult and constantly changing time our nurse-led residence is working daily with Public Health and Humber Regional Hospital IPAC team to insure we are meeting all standards of infection control. All staff and family visitors are given a temperature and rapid test before entering the community and provided with PPE worn in the community. Our residents can assign one essential outside visitor per week who are provided with PPE and meet with the resident in a private family meeting area. With additional cleaning staff and our one to 3 staff ratio our residents are able to spend their time in the community safely distanced with our staff insuring the surfaces they touch are disinfected. We able to safely offer a wide variety of activities to our residents maintaining infection control protocol. As protocol changes we are keeping our families apprised and updated.
